vacation. Numerous summer projects are
underway, including plans for the start of the 2011-2012 school year. To facilitate a smooth opening of school, it is necessary that certain information be on file before the school year begins. All the forms listed below can be found in one file in the column to the right, or you may access each form individually via the links below. Upon request, the front office will mail a printed copy; simply call us at 904.396.5751.
Parents or legal guardians of all students must complete and return the necessary forms to the Front Office by August 5, unless otherwise noted, with all necessary signatures. To help you with this process, we've created a PDF checklist available for download listing all of the forms with instructions for completing
them.
Required forms for 2011-2012 include:

2011-2012 Student Permission Form: This form consists of two specific authorizations; medical treatment and counseling in the event of a medical emergency and attendance and transportation for school-sponsored trips, etc.

2011-2012 Student Photo, Media, Social Media and Video Release Form: This form consists of authorization to use photo, video or other media of students collected by the School or a third party in any School publication or communication.

2011-2012 Health Information Form: This form provides the School a way to respond quickly in the event of a student's illness. It also provides a current record of your child’s medical information, including contact numbers for the parent or legal guardian. It also authorizes the School clinic to dispense or withhold specific medications.

2011-2012 Bullying, Hazing and Harassment Policy: This form is the affirmation of agreement to the School's policy on bullying, hazing and harassment. (Only new students to Episcopal and 9th graders must complete this form)

2011-2012 Network Acceptable-Use Policy: This form is the affirmation of agreement to the School's policy on acceptable-use of the Episcopal Network.
(Only new students to Episcopal and 9th graders must complete this form)- 2011-2012 Grandparent Information Form: This form puts grandparents on mailing lists
for special events, including Grandparents’ Day as well as The Magazine of Episcopal.
- FHSAA Preparticipation Physical Evaluation (Form EL2): This form must be dated after June 1, 2011.
- FHSAA Consent and Release from Liability Certificate (Form EL3): This form must be
dated after June 1, 2011.
